New function modify_xml_document()

This is now exported in place of dom_add_element(), which becomes private.
Explicitly include XML::LibXML and XPathContext, so that this module
	can be used standalone without ZOOM::IRSpy.
Define $IRSPY_NS, which replaced $ZOOM::IRSpy::irspy_ns.
